ABSTRACT

Teleworking has spread drastically during the COVID-19 pandemic, but its effect on musculo-skeletal disorders (MSD) remains unclear. We aimed to make a qualitative systematic review on the effect of teleworking on MSD. Following the PRISMA guidelines, several databases were searched using strings based on MSD and teleworking keywords. A two-step selection process was used to select relevant studies and a risk of bias assessment was made. Relevant variables were extracted from the articles included, with a focus on study design, population, definition of MSD, confounding factors, and main results. Of 205 studies identified, 25 were included in the final selection. Most studies used validated questionnaires to assess MSD, six considered confounders extensively, and seven had a control group. The most reported MSD were lower back and neck pain. Some studies found increased prevalence or pain intensity, while others did not. Risk of bias was high, with only 5 studies with low/probably low risk of bias. Conflicting results on the effect of teleworking on MSD were found, though an increase in MSD related to organizational and ergonomic factors seems to emerge. Future studies should focus on longitudinal approaches and consider ergonomic and work organization factors as well as socio-economic status.

ABSTRACT

OBJECTIVE: In the course of the COVID-19 pandemic the format of education was forced to change from formal to distance in a considerably short time. The study aimed to analyze the changes in the prevalence and the severity of neck pain among teachers during the pandemic. SUBJECTS AND METHODS: The study analyzed the Neck Disability Index (NDI), a self-report questionnaire designed to assess the individual's neck pain experience in daily life and rate a disability score due to neck pain, and numerical rating scale (NRS) report pairs completed by teachers. RESULTS: The mean age of the participants was 41.67±9.57. The majority were female (67.9 %) and were employed for less than 20 years in teaching (68.9%). The survey results collected at the beginning and at the end of the first year of the pandemic showed that: the mean session hours per week were 21.19±9.21 and 21.61±9.45, the mean NDI scores were 11.61±6.17 and 12.65±7.76, and the mean NRS scores were 3.05±2.96 and 4.75±3.13. The female participants scored significantly higher disability scores (p<0.001). The NRS scores were increased in both genders (p<0.001). The NDI scores, the disability grouping, and NRS were significantly associated with weekly session hours (p=0.011, p<0.001 and p<0.001, respectively). The NRS scores were increased in all age groups (p<0.001). The increase in weekly session hours was related to the NRS scores (p<0.001). CONCLUSIONS: In one year during the pandemic, despite unchanged telework hours, a significant increase in neck pain incidence and pain severity was noted among teachers. Unexperienced employees in the line of distance working should receive adequate training to avoid not only musculoskeletal disorders but other physical and psychological unwanted effects.

ABSTRACT

BACKGROUND: Following the declaration of the COVID-19 pandemic by the WHO, many people in the public sector have switched to remote work, including teachers. This could have contributed to the occurrence of back pain in this group. The aim of the study was to assess back pain in a group of teachers before and after the introduction of distance learning due to the COVID-19 pandemic. MATERIAL AND METHODS: The study was based upon an online diagnostic survey and included 361 teachers and university lecturers from 16 provinces of Poland. In order to assess the intensity of pain, a Numerical Rating Scale (NRS) was used. Statistical analysis was performed using IBM SPSS Statistics 26 and Microsoft Excel 2021. The level of statistical significance was adopted on the level p < 0.05. RESULTS: Among the surveyed teachers, the mean degree of declared cervical and lumbar spine pain increased significantly after the COVID-19 pandemic (p < 0.001). The mean value of the declared degree of cervical pain increased from 2.49±2.81 before the pandemic to 5.44±1.83 during the pandemic. Before the pandemic, the mean low back pain intensity was 2.81±2.73 on the NRS, while after the introduction of remote working, it was 5.53±2.20. CONCLUSIONS: There is a relationship between the occurrence of back pain and the change in the nature of work in the group of teachers in connection with the declaration of the COVID-19 pandemic. There has been a statistically significant increase in teachers' back pain intensity since the introduction of distance learning. Depending on the number of hours spent at the computer, the average degree of the declared back pain in the study group increased statistically significantly. There is a need to set the goals of physioprophylaxis and introduce education in the field of ergonomics of computer work in a group of teachers. Med Pr. 2021;72(6):677-84.

ABSTRACT

We defined the prevalence of neck pain, trismus, or dysphagia (28.4%) and retropharyngeal edema (2.9%) among 137 patients with multisystem inflammatory syndrome in children (MIS-c). Retropharyngeal edema or phlegmon has been documented radiologically in at least 9 children. Symptoms of neck inflammation are common in MIS-c.

ABSTRACT

BACKGROUND: The current sanitary crisis brought on by the COVID-19 recently forced a large proportion of workers to adopt telecommuting with limited time to plan transition. Given that several work-related risk factors are associated with headache and neck pain, it seems important to determine those associated with headache and neck pain in telecommuters. The main objective of this study was to identify which telecommuting and individual associated factors are related with headache and neck pain occurrence in telecommuters over a five days follow-up. The second objective was to evaluate the impact of wearing a headset on headache and neck pain intensity in telecommuters. METHODS: One hundred and sixty-two participants in telecommuting situation were recruited. Baseline assessment included sociodemographic data, headache and neck pain-related disability (6-item Headache Impact Test (HIT-6) and Neck Bournemouth Questionnaire (NBQ)), headache and neck pain frequency and intensity as well as questions about the wearing of a headset (headset wearing, headset type and headset wearing hours). A prospective data collection of headache, neck pain and headset wearing was conducted using daily e-mail over a 5-day follow-up. A stepwise multivariate regression model was performed to determine associated factors of headache or neck pain occurrence during the follow-up. A t-test was conducted to assess the impact of headset wearing on headache and neck pain intensity during the follow-up. RESULTS: Regarding headache, the stepwise multivariate regression model showed that the HIT-6 score was associated with future headache occurrence in telecommuters (OR (95% CI) = 1.094 (1.042-1.148); R2 = 0.094; p <  0.001). For neck pain, the stepwise multivariate regression showed that the NBQ score was related to future neck pain occurrence in telecommuters (OR (95% CI) = 1.182 (1.102-1.269); R2 = 0.182; p <  0.001). T-test showed no difference between participants that wore a headset and participant that did not wore a headset on mean headache (p = 0.94) and neck pain (p = 0.56) intensity during the five days follow-up. CONCLUSION: Although several work-related risk factors are associated with headache and neck pain in workers, telecommuting did not present the same risks. Working set-up did not have a significant impact on headache and neck pain as headache-related disability was the only associated factor of future headache episodes and neck-pain related disability was the only associated factor of future neck pain episodes. Also, wearing a headset had no impact on headache and neck pain in telecommuters.

ABSTRACT

PURPOSE: The aim of this study was to investigate the effect of working from home on neck pain (NP) among office workers during the COVID-19 pandemic. METHODS: Participants from two Swiss organisations, aged 18-65 years and working from home during the lockdown (n = 69) were included. Baseline data collected in January 2020 before the lockdown (office work) were compared with follow-up data in April 2020 during lockdown (working from home). The primary outcome of NP was assessed with a measure of intensity and disability. Secondary outcomes were quality of workstation ergonomics, number of work breaks, and time spent working at the computer. Two linear mixed effects models were fitted to the data to estimate the change in NP. RESULTS: No clinically relevant change in the average NP intensity and neck disability was found between measurement time points. Each working hour at the computer increased NP intensity by 0.36 points (95% CI: 0.09 to 0.62) indicating strong evidence. No such effect was found for neck disability. Each work break taken reduced neck disability by 2.30 points (95% CI: - 4.18 to - 0.42, evidence). No such effect was found for NP intensity. There is very strong evidence that workstation ergonomics was poorer at home. CONCLUSION: The number of work breaks and hours spent at the computer seem to have a greater effect on NP than the place of work (office, at home), measurement time point (before COVID-19, during lockdown) or the workstation ergonomics. Further research should investigate the effect of social and psychological factors. TRIAL REGISTRATION: ClinicalTrials.gov, NCT04169646. ABSTRACT

Evidence about the characterization of home workers in terms of both work-related outcomes and health issues is lacking. The purpose of this cross-sectional study was to examine the impact of home working on perceived job productivity and satisfaction, work-related stress, and musculoskeletal (MSK) issues. We included 51 mobile workers, collecting data about demographic characteristics, working experience, job productivity, and stress. Job satisfaction was assessed through the Utrecht Work Engagement Scale (UWES), while MSK pain was investigated by the Brief Pain Inventory (BPI) and Fear Avoidance Beliefs Questionnaire (FABQ). Moreover, a home workplace analysis had to be carried out according to current Italian regulations. Participants declared that they were less productive (39.2%) but less stressed (39.2%) and equally satisfied (51%) compared to the time of office working. Regarding MSK disorders, low back pain (LBP) was referred by 41.2% of home workers and neck pain by 23.5% of them. Neck pain worsened in 50% of home workers, while LBP did not exacerbate in 47.6% of cases. Home workers with MSK pain reported a lower job satisfaction. Depending on our data, the home environment seems to be not adequate in the mobile worker population, with an increased risk for mental health and MSK problems, particularly affecting the spine. Addressing these issues can significantly reduce risks for health, thus, improving job productivity and satisfaction and reducing cost.
